Volunteer Form
Thank you for your interest in volunteering at EPS. Prior to volunteering at Eastside Prep, all parents/guardians are required to complete the online volunteer form located HERE.
Reimbursement Form
Parents/guardians wishing to be reimbursed for PA-related expenses can download and complete the form linked below. Completed forms should be submitted to Vickie Baldwin at vbaldwin@eastsideprep.org.
